How does it work?

This medicine contains the active ingredient gliclazide, which is a type of medicine used to help control blood sugar levels in people with type 2 diabetes.

People with type 2 diabetes (non-insulin dependent diabetes) have a deficiency of a hormone called insulin. Insulin is produced by the pancreas and is the main hormone responsible for controlling sugar levels in the blood. It normally makes the cells of the body remove excess sugar from the blood. In type 2 diabetes the cells of the body are also resistant to the action of insulin that is produced, which means that blood sugar levels can become too high.

Gliclazide is a type of antidiabetic medicine known as a sulphonylurea. It helps control blood sugar levels by acting on the cells in the pancreas that produce insulin. These cells are called beta cells, and gliclazide causes them to produce more insulin.

Gliclazide improves insulin production immediately after eating. This is called early or first phase insulin secretion, and does not normally happen in people with type 2 diabetes. The enhanced insulin production results in a blood sugar lowering effect in response to meals or glucose, as occurs naturally in non-diabetic people.

Gliclazide is used when dietary measures, weight loss and physical exercise are not enough to control blood sugar levels in people with type 2 diabetes.

Gliclazide has been shown to prevent blood cells called platelets from clumping together in the blood. It also increases the breakdown of blood clots that form within the blood vessels. This may help prevent the long-term complications of diabetes, which may be partly due to changes in the blood vessels caused by these mechanisms.



What is it used for?

  • Type 2 (non-insulin dependent) diabetes

Warning!

  • If you experience stress such as trauma, surgery, infection or fever while stabilised on an oral antidiabetic medicine, your blood sugar control may be reduced or lost. In these cases your doctor may temporarily replace your treatment with insulin.
  • This medicine can cause low blood sugar levels (hypoglycaemia). It is important that you take precautions to avoid hypoglycaemia when driving or operating machinary. You should also be aware that the risk of hypoglycaemia is increased by irregular food intake, strenuous physical exercise and drinking alcohol. It is important that you are aware of the warning signs of hypoglycaemia and know what to do if you experience them. You should discuss your blood sugar control with your doctor, pharmacist or diabetic nurse to make sure you get the best out of your treatment.

Use with caution in

  • Decreased kidney function
  • Decreased liver function
  • Elderly people
  • Hypothyroidism
  • Insufficient production of hormones by the pituitary gland (pituitary insufficiency)
  • Insufficient production of natural steroid hormones by the adrenal glands (adrenal insufficiency)
  • Malnutrition
  • Severe coronary artery disease

Not to be used in

  • Allergy to medicines from the sulphonamide group, eg the antibiotic sulphamethoxazole
  • Allergy to sulphonylureas such as gliclazide, glibenclamide or tolbutamide
  • Breastfeeding
  • Coma due to ketoacidosis in severe and inadequately treated diabetes (diabetic coma)
  • Diabetic keto-acidosis
  • Life long inherited blood diseases which can cause a variety of symptoms, including mental health problems (porphyrias)
  • Pregnancy
  • Severely decreased kidney function
  • Severely decreased liver function
  • Type 1 (insulin dependent) diabetes

This medicine should not be used if you are allergic to one or any of its ingredients. Please inform your doctor or pharmacist if you have previously experienced such an allergy.

If you feel you have experienced an allergic reaction, stop using this medicine and inform your doctor or pharmacist immediately.

Pregnancy and Breastfeeding

Certain medicines should not be used during pregnancy or breastfeeding. However, other medicines may be safely used in pregnancy or breastfeeding providing the benefits to the mother outweigh the risks to the unborn baby. Always inform your doctor if you are pregnant or planning a pregnancy, before using any medicine.

  • This medicine should not be used during pregnancy. Control of diabetes during pregnancy is usually achieved using insulin, as insulin provides a more stable control of blood sugar. Discuss this with your doctor.
  • It is not known whether this medicine passes into breast milk. It should not be used by breastfeeding women because of the potential harm it may cause to nursing infants. Seek medical advice from your doctor.

Side effects

Medicines and their possible side effects can affect individual people in different ways. The following are some of the side effects that are known to be associated with this medicine. Because a side effect is stated here, it does not mean that all people using this medicine will experience that or any side effect.

  • Visual disturbances
  • Low blood glucose level (hypoglycaemia)
  • Indigestion (dyspepsia)
  • Yellowing of the skin and eyes (jaundice)
  • Inflammation of the liver (hepatitis)
  • Skin reactions such as rash and itch
  • Disturbances in the normal levels of blood cells in the blood
  • Disturbances of the gut such as diarrhoea, constipation, nausea, vomiting or abdominal pain

How can this medicine affect other medicines?

Tell your doctor or pharmacist what medicines you are taking, including those bought without a prescription and herbal medicines, before you start treatment with this medicine. Likewise, always check with your doctor or pharmacist before taking any new medicines during treatment with this one.

The following medicines may enhance the blood sugar lowering effect of gliclazide. This may cause low blood sugar (hypoglycaemia) and this may happen unpredictably:
- the antifungal medicine miconazole (miconazole should not be taken with gliclazide)
- other antidiabetic medicines
- ACE inhibitors, eg captopril, enalapril
- anticoagulants, eg warfarin
- large doses of aspirin (small pain-killing doses do not normally have this effect)
- beta-blockers, eg propranolol (plus beta-blockers may mask the signs of low blood sugar, such as increased heart rate and tremor)
- cimetidine
- fibrates, eg clofibrate
- disopyramide
- monoamine oxidase inhibitor (MAOI) antidepressants, eg phenelzine
- non-steroidal anti-inflammatory drugs (NSAIDs), eg phenylbutazone, azapropazone
- sulphonamide antibiotics, eg co-trimoxazole
- tetracycline antibiotics, eg oxytetracycline, doxycycline.

The following medicines may increase blood sugar levels and therefore oppose the blood sugar lowering effect of gliclazide:
- chlorpromazine
- corticosteroids, eg prednisolone
- danazol
- oral contraceptives
- thiazide diuretics, eg bendrofluazide.

Gliclazide may increase the anti-blood-clotting effects of anticoagulant medicines such as warfarin.
